HEARTWORM Preventative NOTICE: If your order includes any heart worm preventatives ONE of the following MUST be provided.

•Your pet is under 6 months of age (receipt of last vet check up indicating estimated age).
• Your dog was tested for heartworm at ACT within the last year.
• You have faxed a prescription for heartworm preventative from your general veterinarian.(813-920-8353) .                                                                                                                           

COMFORTIS flea preventive NOTICE:  This order also needs a prescription from either our veterinarian (for our clients) or from yours.  This is a new product and very popular.  Very few side effects have been reported!
